This is a way to refer to the United States. It is rarely used within U.S. borders, but is fairly common in other countries. This expression comes from the fact that the second largest geographical/political unit in the US is the state, and that these states are united under a federal government. This mattered alot more originally, when states were more like independant nations, but they have little autonomy now.
